Here's the concern with pairing Andrographis and your tacrolimus. Tacrolimus works by quieting down your immune system so it won't attack your transplanted organ. Andrographis seems to do the opposite, lab and animal studies hint that it can rev the immune system back up. So in theory, the herb could partly work against what tacrolimus is trying to do, and that could nudge you toward the thing we most want to avoid after a transplant: organ rejection.

I want to be honest that this is theoretical and based only on test-tube and animal research, not real patients. But tacrolimus is a drug where even small shifts matter, so I'd play it safe. Please don't start Andrographis without first checking with your transplant team or pharmacist.

Tacrolimus is prescribed to help prevent your body from rejecting a transplanted organ. It is used in adults who receive a kidney, liver, or heart transplant, and in children who receive a liver transplant. It is always used alongside other anti-rejection medicines.

Because tacrolimus works by dampening your immune system, it lowers the risk of your body attacking the new organ — but it also means you need to be watched closely for infections and other side effects throughout your treatment.
