Animal research suggests that neem might have immunostimulant effects.

Here's the gist with denosumab (you may know it as Prolia or XGEVA). This shot works on your immune cells to slow down the cells that break down bone. The concern on file is that neem might give your immune system a little nudge in the opposite direction. In animal studies, neem appeared to stimulate immune activity, so in theory it could push back against medicines that calm the immune system.
I want to be honest about how thin this evidence is. It comes from lab and animal work, not from people taking denosumab, so we don't actually know if it changes how well your bone treatment works. I wouldn't lose sleep over it, but since denosumab is an important injection for your bones, run your neem product past your pharmacist or doctor before combining them.

Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. About Denosumab Injection
What Is Denosumab Injection Used For?
Brand names: Prolia, XGEVA
Jubbonti (denosumab) injection is used to treat osteoporosis in postmenopausal women and men at high risk for fracture, including those who have already had a fracture or who have not responded to other osteoporosis medicines. It is also used to treat osteoporosis caused by long-term steroid (glucocorticoid) use in men and women.
Jubbonti is additionally used to increase bone mass in men receiving androgen deprivation therapy (a hormone-lowering treatment) for nonmetastatic prostate cancer, and in women receiving aromatase inhibitor therapy for breast cancer — both of which can weaken bones over time.
