Tocilizumab falls under IMMUNOSUPPRESSANTS — the group Elderberry interacts with.
Elderberry has immunostimulant activity, increasing the production of cytokines, including interleukin and tumor necrosis factor.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with elderberry and Actemra (tocilizumab). Tocilizumab works by quieting down the immune system, specifically by blocking a signaling molecule called interleukin-6 (IL-6), which helps calm the inflammation behind rheumatoid arthritis and similar conditions. Elderberry, on the other hand, may nudge the immune system in the opposite direction by boosting certain cytokines like IL-6 and TNF. So in theory, the supplement could work against what your medication is trying to do, possibly making your treatment a little less effective.
I want to be honest here, though. This is mostly a theoretical concern from lab studies, not something firmly proven in people taking tocilizumab. It's still worth running past your rheumatologist or pharmacist before starting elderberry, just so you don't risk your arthritis control slipping.

Tocilizumab injection is used to treat several serious inflammatory and immune-related conditions. Actemra (tocilizumab) is indicated for moderately to severely active rheumatoid arthritis in adults who haven't responded well to other disease-modifying drugs, giant cell arteritis in adults, systemic sclerosis-associated interstitial lung disease (SSc-ILD) to slow lung function decline, polyarticular and systemic juvenile idiopathic arthritis in patients 2 years and older, and severe or life-threatening cytokine release syndrome (CRS) caused by CAR T-cell therapy in adults and children 2 and older.
Actemra is also approved for hospitalized adults and children aged 2 and older with COVID-19 who are on corticosteroids and need supplemental oxygen or mechanical breathing support.
