Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that Bulbine natalensis stem extract slightly induces CYP1A2 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP1A2 substrates; however, this has not been studied in humans.

Here's the gist with Bulbine natalensis and your Pomalyst (pomalidomide). Pomalidomide is partly cleared by a liver enzyme called CYP1A2. In lab (test tube) studies, this plant appeared to gently speed that enzyme up. If that happened in the body, your pomalidomide could be broken down a bit faster, meaning lower drug levels and possibly less benefit against your multiple myeloma or Kaposi's sarcoma.

I want to be honest about the strength of this, though. It's theoretical and based only on lab data, never tested in people, so we can't say it truly matters in real life. Still, with a cancer therapy like this one, I'd play it safe. Please check with your oncologist or pharmacist before adding this supplement, especially since it has raised liver and kidney concerns too.

Here's the concern with pairing this supplement with Pomalyst (pomalidomide). Your body partly clears pomalidomide using a liver enzyme called CYP3A4. In lab studies (not in people), Bulbine natalensis seemed to speed that enzyme up. If that happened in your body, it could clear pomalidomide faster and leave less of it working against your myeloma or Kaposi's sarcoma, which is the opposite of what you want during cancer treatment.

I want to be honest: this is theoretical and only based on test-tube data, so we don't truly know if it matters in humans. Still, with a serious cancer drug, I wouldn't gamble. There are also animal reports of liver and kidney harm with this supplement. Please run it by your oncology team or pharmacist before adding anything.

Here's the gist: Pomalidomide (Pomalyst) is moved around your body partly by a transporter called P-glycoprotein, which acts like a pump that escorts the drug in and out of cells. The concern on file is that Bulbine natalensis may nudge that pump's activity, which could in theory change how much pomalidomide ends up in your bloodstream.

I want to be honest about how thin this evidence is. It comes only from lab (in vitro) work, not from people, so we really don't know if it matters in real life. Still, pomalidomide is serious cancer medicine with effects like low blood counts, fatigue, and infection risk, so I wouldn't gamble. Please don't start Bulbine natalensis without first running it past your oncology team or pharmacist.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
