Here's the idea behind this one. Elderberry can gently rev up parts of your immune system, nudging it to make more signaling chemicals like interleukins and tumor necrosis factor. Pazopanib (Votrient) is grouped with medicines that calm or redirect immune and cell-growth activity, so in theory elderberry could pull in the opposite direction and work against what your treatment is trying to do.

I want to be honest about the strength of this: it's mostly theoretical, based on lab signals rather than solid studies in people taking pazopanib. With a cancer therapy, though, I lean cautious. Before adding an elderberry supplement, please run it by your oncologist or pharmacist first. Occasional culinary amounts in a cooked product are less concerning than daily concentrated capsules or syrups.

Theoretically, elderberry might interact with pazopanib, potentially increasing the risk of adverse effects.
In one case, a 65-year-old patient taking pazopanib for 4 weeks for soft-tissue sarcoma developed severe nausea, loose stools, and elevated alanine transaminase and aspartate transaminase levels and was diagnosed with grade 3 liver injury. The patient reported taking elderberry supplements for 2 years, dose unspecified, and was also taking a multivitamin and a calcium supplement. The patient's symptoms and liver enzyme levels improved upon discontinuation of pazopanib and all supplements. Pazopanib treatment was re-initiated, at a lower dose, with no further evidence of liver injury. It was unclear in this case report if the elderberry supplements were derived from the berry, the flower, or the combination.

Here's the honest picture: the worry about combining elderberry with pazopanib (Votrient) rests on very thin evidence, basically a single case report. In that one situation, a patient on pazopanib who had been taking elderberry for years developed significant liver injury, with nausea, loose stools, and elevated liver enzymes. Things improved once both were stopped.

Pazopanib is already known to stress the liver on its own, so it's hard to say elderberry truly caused this. Still, since both could theoretically tax the liver, it's reasonable to be cautious. Pazopanib is an active drug (not a prodrug), so this is about possible added side effects, not lost effectiveness. Please don't start or stop anything yourself. Loop in your oncology team or pharmacist, since they already watch your liver labs closely.

Elderberry is a popular herbal supplement, mainly taken to help with colds and flu. Some small studies suggest it may modestly shorten cold or flu symptoms, but the evidence is limited and not conclusive. Cooked berries and properly prepared products are generally well tolerated, but raw or unripe parts can cause nausea and should be avoided.

What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
