These probiotics are generally well tolerated in healthy adults and children. However, because they contain live bacteria, there's a small risk of infection in people with weakened immune systems — especially those who are critically ill or have had recent surgery.

Mild gastrointestinal side effects like bloating, flatulence, abdominal discomfort, and diarrhea have been reported, though these are uncommon. In rare cases, particularly in very sick or immunocompromised patients, these bacteria have caused serious infections like sepsis and bacteremia; most reported cases have involved critically ill infants or people with underlying heart or immune conditions.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
