The evidence for both active ingredients is limited. Graviola has been studied for human papillomavirus (HPV), headache, and use as an insect repellent, as well as male infertility, but we do not have sufficient reliable evidence to rate it for any of these.
Agaricus mushroom has been studied for allergic rhinitis (hay fever), osteoporosis, cancer, cardiovascular disease, chemotherapy side effects, and Crohn disease, but again the reliable evidence is insufficient to rate it. Neither ingredient has established effectiveness in our data.
How safe is it?
Well-documented data
Both ingredients carry safety cautions. Graviola has limited safety data and lab studies raise concerns about possible nerve toxicity with long-term use.
The most common side effects from graviola are stomach pain and nausea. Rare but serious effects include movement disorders and nerve damage (myeloneuropathy) — symptoms similar to Parkinson disease.
People with Parkinson disease should avoid it, as even small amounts may worsen motor and cognitive symptoms. Agaricus mushroom is generally well tolerated as a food, but supplement safety and long-term effects are not well established.
Most common side effects are abdominal discomfort, diarrhea, and nausea. Rare serious effects include allergic reactions, liver damage, and interstitial lung disease (a lung condition).
Graviola is rated possibly unsafe in pregnancy and should be avoided while breastfeeding. Agaricus mushroom should be avoided at supplement doses during pregnancy and breastfeeding — not enough safety data exists for either.

Agaricus brasiliensis
Antidiabetes Drugs
Theoretically, taking agaricus mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
In one clinical study in patients with type 2 diabetes who are stabilized on conventional oral hypoglycemic agents, 3 of 29 patients taking an agaricus mushroom extract 500 mg three times daily for 12 weeks reported hypoglycemia, compared to one of 29 patients in the placebo group.
Graviola
Carbamazepine (Tegretol)
Theoretically, graviola might decrease concentrations of carbamazepine when used concomitantly.
A study in rats shows that graviola extract reduces the area under the curve of carbamazepine by 46% and maximum concentration by 35% when compared with carbamazepine alone.
