The evidence behind glutamine is strongest for sickle cell disease, where it's considered effective. For other uses — HIV/AIDS-related wasting, recovery from surgery, and trauma or critical illness — the data shows it's possibly effective, meaning there's some promise but the evidence isn't as solid yet.
How safe is it?

Glutamine is generally well tolerated in healthy adults. The most common side effects from oral use are digestive: belching, bloating, constipation, diarrhea, flatulence, nausea, vomiting, and stomach pain.
Headache and musculoskeletal pain have also been reported. A couple of isolated cases of dizziness, mania, or hypomania have been noted, usually at very high doses or in people with bipolar disorder.
If you have kidney or liver disease, use glutamine only under your doctor's supervision. There isn't enough safety data to recommend it during pregnancy as a supplement dose, though the evidence suggests it's likely safe — talk to your pharmacist or doctor about your specific situation.
