How safe is it?
Well-documented data
Moringa leaf powder appears to be generally well tolerated when used as a food, but high-quality safety data are limited. The most common side effect is transient diarrhea—in one trial, about 25% of people taking 8 grams daily experienced it.
Rare allergic reactions, including anaphylaxis, have been reported, and there is one case report of Stevens-Johnson syndrome (a serious skin reaction) linked to moringa leaf consumption. The root and root bark are concerning—animal research suggests they may stimulate the uterus, and safety data are lacking, so medicinal amounts should be avoided in pregnancy.
Moringa leaf is traditionally used to boost breast milk, but safety in lactation has not been well studied, so caution is advised.

Herbal Hills Moringa contains shigru (moringa) leaf powder, which has documented interactions with several medication types.
The most serious concern is with drugs metabolized by CYP3A4 enzymes—a large group that includes many common medications—where moringa may unpredictably raise or lower drug levels because in vitro research shows conflicting effects (some studies suggest it blocks the enzyme, others suggest it speeds it up). A study in HIV patients found no change with one specific drug, but the theoretical risk remains for others in this class.

Moderate interactions are also documented with levothyroxine (thyroid medication), where moringa leaf may reduce how well your body converts the active form of the hormone, potentially weakening the drug's effect. P-glycoprotein substrate drugs—a transport system your kidneys use—may also have their levels increased by moringa leaf extract, though this has not yet been seen in humans.
Minor interactions are listed for blood sugar medications (animal studies show moringa lowers glucose, but human studies are mixed) and CYP1A2 substrate drugs, where moringa may decrease their effectiveness.
Although nevirapine (an HIV medication) theoretically could interact through CYP3A4, a real patient study showed no actual change in drug levels. Shigru (leaves) powder
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Levothyroxine (Synthroid, Others)
Theoretically, moringa leaf can antagonize the effects of levothyroxine.
Animal research suggests that moringa aqueous leaf extract might reduce serum triiodothyronine (T3) concentrations by inhibiting the peripheral conversion of thyroxine (T4) to T3.
P-Glycoprotein Substrates
Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Antidiabetes Drugs
Theoretically, moringa might have additive effects when used with antidiabetes drugs; however, research is conflicting.
Animal research shows that moringa can lower blood glucose levels. However, research in humans has not shown consistent blood glucose lowering effects.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, moringa might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that moringa extract induces CYP1A2 enzymes.
Nevirapine (Viramune)
Moringa leaf is unlikely to have a clinically significant interaction with nevirapine.
Nevirapine is partially metabolized by cytochrome P450 3A4 (CYP3A4). In vitro evidence suggests that moringa inhibits CYP3A4. However, a pharmacokinetic study in patients with HIV shows no change in nevirapine pharmacokinetics when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
