CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Let me put this one at ease for you. These cold and flu combo products contain three active ingredients, and the one most affected by the liver enzyme CYP3A4 is the cough suppressant dextromethorphan (it's actually handled mostly by a different enzyme, CYP2D6, so even here the link is loose). The lab data on moringa is honestly all over the place: some test-tube studies say it slows CYP3A4 down, others say it speeds it up. So we can't even confidently say which direction it would go.
Here's the reassuring part: this is theoretical, based only on lab and animal work, and a human study found no real change with another CYP3A4 medication. With everyday food amounts of moringa, I wouldn't lose sleep. If you take concentrated moringa capsules daily, just mention it to your pharmacist so we can keep an eye on things.

This combination cold and flu product (acetaminophen for pain and fever, dextromethorphan for cough, and pseudoephedrine for congestion) is one a lot of folks reach for during cold season. The concern here is pretty mild. Lab studies suggest moringa may speed up an enzyme called CYP1A2, which helps process certain medications. In theory, that could clear some of these ingredients a little faster, meaning they might wear off sooner and feel slightly less effective.
The honest truth is this is based only on test-tube research, not real people, so I wouldn't lose sleep over it. Food amounts of moringa are very unlikely to matter, and even supplement doses probably won't cause a noticeable change. If you use moringa regularly, just mention it to your pharmacist so they can keep an eye on things.
