Mushroom Immune contains five active mushroom extracts: beta-glucan (a polysaccharide from mushrooms), chaga, maitake, shiitake, and reishi—each included as a powder or extract. There are also two inactive ingredients: hydroxypropylmethylcellulose (the capsule material) and magnesium stearate (a processing aid).

The evidence for what these mushrooms can do is mixed. Beta-glucan is likely effective for lowering cholesterol (dyslipidemia) and supporting heart health in coronary heart disease.
For the other four mushroom extracts—chaga, maitake, shiitake, and reishi—the data we hold doesn't establish their effectiveness for most conditions. How safe is it?
Well-documented data
Beta-glucan from food is generally well tolerated, but concentrated supplement doses lack safety research, so caution is advised. Chaga has two reported cases of kidney failure from excessive consumption due to its high oxalate content.
Maitake and shiitake are generally well tolerated when used as food, but concentrated supplements haven't been fully studied; shiitake can cause gastrointestinal upset (diarrhea, nausea, abdominal discomfort) in some people, and raw shiitake can trigger a skin condition called shiitake dermatitis. Reishi is generally well tolerated short-term but may cause dizziness, dry mouth, nausea, rash, or stomach upset.
Long-term safety for reishi isn't well established. None of these mushroom extracts have enough safety data during pregnancy or breastfeeding; the facts advise against use during both.

Beta-glucan and maitake mushroom can both lower blood pressure, so taking either with blood pressure medications (antihypertensives) might cause your pressure to drop too far. Maitake also has a documented case of increasing the blood-thinning effect of warfarin; it raised one patient's INR (a measure of how thin the blood is) to a level that needed attention.
Chaga and reishi mushroom carry similar theoretical risks with blood thinners (anticoagulant or antiplatelet drugs)—chaga through platelet effects seen in the lab, reishi at higher doses.
Chaga, maitake, and reishi mushroom can all theoretically lower blood sugar when combined with diabetes medications (antidiabetes drugs), raising the risk of low blood sugar (hypoglycemia). Beta-glucan, chaga, maitake, and shiitake mushroom may interfere with immune-suppressing medications (immunosuppressants) used after organ transplants or for certain autoimmune conditions.
Shiitake mushroom might also affect how your body breaks down certain psychiatric and heart medications that rely on a liver enzyme called CYP2D6.

Mushroom Immune Complex
Antidiabetes Drugs
Theoretically, combining maitake mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that taking maitake mushroom polysaccharide (MMP) can lower blood glucose levels in patients with types 2 diabetes.
Antihypertensive Drugs
Theoretically, combining maitake mushroom with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that maitake mushroom can lower blood pressure.
Warfarin (Coumadin)
There is limited evidence that maitake mushroom may increase the anticoagulant effects of warfarin.
In a case report, a patient previously stabilized on warfarin developed an elevated international normalized ratio (INR) of 5.1 after taking maitake mushroom (Grifron-Pro Maitake D-Fraction) 1 drop/kg daily in three divided doses for one week. The elevated INR resolved after holding warfarin for two days, then reducing the dose by 11%. It is thought that the beta-glucan constituent of maitake mushroom might cause warfarin dissociation from proteins, resulting in increased free warfarin levels and increased warfarin effects.
