Blond psyllium in this formula has the strongest evidence: it's effective for constipation and likely effective for high cholesterol and heart disease. Acidophilus is possibly effective for irritable bowel syndrome, Helicobacter pylori infection, antibiotic-related diarrhea, and bacterial vaginosis, though the evidence is modest.
Citrus pectin is possibly effective for high cholesterol. Yogurt cultures are possibly effective for high cholesterol, lactose intolerance, and constipation.

Whey powder is generally well tolerated by most healthy adults, though dose-related side effects include acne, bloating, cramps, diarrhea, nausea, and reflux; it's not well studied as a concentrated supplement in pregnancy or lactation. Blond psyllium is generally well tolerated with adequate water intake; common effects are abdominal pain, diarrhea, gas, and nausea, and rare serious effects include bowel obstruction; it's likely safe in pregnancy.

The most serious interaction is between whey powder and levodopa (a Parkinson's medication), where protein can worsen tremor, rigidity, and symptom fluctuations—a Major-severity concern.

Several Moderate-severity interactions affect antibiotics: acidophilus, citrus pectin, whey powder, and yogurt cultures can all reduce antibiotic effectiveness. Pectin may also reduce the absorption of lovastatin (a cholesterol drug) and digoxin (a heart medication).
Whey powder additionally interacts with bisphosphonates (bone drugs) and quinolone antibiotics by binding minerals that interfere with their absorption. Blond psyllium can alter absorption of metformin (diabetes), lithium (mood stabilizer), carbamazepine (seizure), and olanzapine (antipsychotic), and may affect digoxin levels.

Blond Psyllium
Carbamazepine (Tegretol)
Theoretically, blond psyllium might reduce the effects of carbamazepine and increase the risk for convulsions.
Blond psyllium might reduce carbamazepine absorption and serum levels. A preliminary study using blond psyllium reported decreased carbamazepine bioavailability due to binding of the drug to psyllium, as well as reduction of available fluid in the gut for dissolution of the drug.
Lithium
Theoretically, taking blond psyllium at the same time as lithium might reduce lithium absorption.
The fiber in blond psyllium might decrease the absorption of lithium. Some case reports describe a reduction in plasma lithium levels with concomitant administration of blond psyllium. This was reversed when psyllium was stopped.
Metformin (Glucophage)
Theoretically, blond psyllium might increase the therapeutic and adverse effects of metformin.
Concurrent use of blond psyllium with metformin slows and increases metformin absorption. To avoid changes in absorption, take psyllium 30-60 minutes after metformin.
Olanzapine (Zyprexa)
Theoretically, taking blond psyllium at the same time as olanzapine might reduce olanzapine absorption.
The fiber in blond psyllium might decrease the absorption of olanzapine. A single case report describes a reduction in the effectiveness of olanzapine when it was taken concomitantly with an unspecified type of psyllium 3 grams orally twice daily. This effect was reversed when psyllium was stopped.
Digoxin (Lanoxin)
Theoretically, taking blond psyllium at the same time as digoxin might reduce digoxin absorption.
Psyllium might bind digoxin in the gut. However, some clinical evidence suggests that psyllium does not impact digoxin absorption.
Ethinyl Estradiol
Theoretically, taking blond psyllium at the same time as ethinyl estradiol might alter levels of estradiol.
Concurrent use of blond psyllium with ethinyl estradiol results in a slight increase in the extent of ethinyl estradiol absorption and a slower rate of absorption. However, this is unlikely to be clinically significant.
Oral Drugs
Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.
Acidophilus
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Whey powder
Levodopa
Theoretically, whey protein might decrease levodopa absorption.
Small clinical studies show that concomitant ingestion of protein or high doses of leucine or isoleucine (100 mg/kg) and levodopa can exacerbate tremor, rigidity, and the "on-off" syndrome in patients with Parkinson disease.
Bisphosphonates
Theoretically, whey protein might reduce the absorption of bisphosphonates.
Whey protein contains minerals such as calcium that bind bisphosphonates in the gut. Advise patients to take bisphosphonates at least 30 minutes before whey protein, but preferably at a different time of day.
Quinolone Antibiotics
Theoretically, whey protein might decrease quinolone absorption.
Whey protein contains minerals, such as calcium, that can bind to quinolones in the gut. To avoid this interaction, advise patients to take oral quinolones at least 2 hours before or 4-6 hours after whey protein.
Tetracycline Antibiotics
Theoretically, whey protein might decrease tetracycline absorption.
Whey protein contains minerals, such as calcium, that bind to tetracyclines in the gut. To avoid this interaction, advise patients to take oral tetracyclines at least 2 hours before or 4-6 hours after whey protein.
Citrus Pectin
Digoxin (Lanoxin)
Theoretically, pectin might reduce the absorption of digoxin, potentially decreasing its effectiveness.
A small clinical study shows that taking digoxin with a kaolin-pectin suspension reduces the absorption of digoxin by about 62%. It is unclear if these effects are due to pectin, kaolin, or the combination.
Lovastatin (Mevacor)
Theoretically, pectin might reduce the absorption of lovastatin, potentially decreasing its effectiveness.
Case reports suggest that concomitant use of pectin and lovastatin might reduce the cholesterol-lowering effect of lovastatin, possibly due to reduced intestinal absorption of lovastatin.
Tetracycline Antibiotics
Theoretically, pectin might reduce the absorption of tetracycline antibiotics, potentially decreasing their effectiveness.
A small clinical study shows that taking tetracycline with bismuth subsalicylate in a kaolin-pectin suspension reduces the absorption of tetracycline by about 34%. It is unclear if these effects are due to pectin, kaolin, bismuth subsalicylate, or the combination.
Yogurt Cultures
Ciprofloxacin (Cipro)
Taking yogurt with ciprofloxacin might reduce the levels and clinical effects of ciprofloxacin.
A small study in humans shows that concomitant administration of yogurt and ciprofloxacin can significantly reduce absorption of ciprofloxacin.
Tetracycline Antibiotics
Taking yogurt with tetracycline antibiotics seems to reduce the levels and clinical effects of tetracycline antibiotics.
Concomitant administration with yogurt can reduce the absorption of tetracycline antibiotics, likely due to its calcium content.
Nilotinib (Tasigna)
Yogurt might increase the absorption of nilotinib; however, it is unlikely to alter its clinical effects.
Pharmacokinetic research in humans shows that taking nilotinib 400 mg dispersed in 1 teaspoon of yogurt increases absorption of nilotinib by about 31%. However, yogurt does not appear to modify the metabolism of nilotinib or increase the risk of adverse effects.
