Each mushroom brings its own profile. Maitake, cordyceps, and reishi are the ones with the most research backing their use, though even for them the evidence of benefit is mixed or limited depending on what condition you're considering.
Himematsutake, like maitake, comes from the agaricus family.
Does it work?
Not established
The evidence doesn't establish strong benefit for any of these mushrooms in the data we hold. Maitake is rated insufficient evidence for hay fever, chemotherapy-related anemia, laryngeal cancer, chemotherapy diarrhea, and chronic fatigue.
Cordyceps is possibly ineffective for athletic performance, and insufficient evidence otherwise for sexual dysfunction, arrhythmia, asthma, lung cancer, and COPD. Reishi is possibly ineffective for high cholesterol (hyperlipidemia), and insufficient evidence for altitude sickness, Alzheimer disease, and athletic performance.
Himematsutake similarly shows insufficient evidence for hay fever, osteoporosis, cancer, heart disease, chemotherapy side effects, and Crohn disease. What this means: these mushrooms haven't been proven to work for the uses marketed, though that's different from proof they don't work — the research just isn't there yet.
How safe is it?
Well-documented data
Maitake, cordyceps, reishi, and himematsutake are each generally well tolerated when taken by mouth. Stomach upset is the most common side effect across all four — some trial participants reported diarrhea, nausea, abdominal discomfort, or constipation.
A small number had rashes or allergic reactions (reishi can trigger respiratory allergy to mushroom spores). One cordyceps case involved liver inflammation (hepatitis), though it's unclear if the cordyceps itself or a contaminant was responsible.
Reishi has also rarely caused dizziness, headache, and insomnia. During pregnancy and breastfeeding, the safety data is insufficient for all four checked ingredients.
The facts advise avoiding concentrated supplement doses of these mushrooms in pregnancy and while breastfeeding. Talk with your doctor or pharmacist about whether this product is right for you if you are pregnant, planning pregnancy, or nursing.

The most serious interaction is maitake's moderate-severity effect on blood thinners (anticoagulants): a case report showed a patient's warfarin level spiked dangerously after taking maitake mushroom.

Beyond blood thinners, maitake and himematsutake both interact moderately with antidiabetes drugs — they can lower blood sugar, which combined with diabetes medication raises the risk of low blood sugar (hypoglycemia). Maitake and reishi both interact moderately with blood pressure medications, potentially making your pressure drop too far.
Cordyceps interacts moderately with blood thinners and immunosuppressants (drugs that weaken your immune response), and has a minor interaction with testosterone supplements or therapies.
Reishi also interacts moderately with blood thinners at higher doses, though 1.5 grams daily does not appear to affect bleeding. We could not check antrodia camphorata, shiitake, or turkey tail — we hold no data for those ingredients.

Reishi
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
Maitake
Antidiabetes Drugs
Theoretically, combining maitake mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that taking maitake mushroom polysaccharide (MMP) can lower blood glucose levels in patients with types 2 diabetes.
Antihypertensive Drugs
Theoretically, combining maitake mushroom with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that maitake mushroom can lower blood pressure.
Warfarin (Coumadin)
There is limited evidence that maitake mushroom may increase the anticoagulant effects of warfarin.
In a case report, a patient previously stabilized on warfarin developed an elevated international normalized ratio (INR) of 5.1 after taking maitake mushroom (Grifron-Pro Maitake D-Fraction) 1 drop/kg daily in three divided doses for one week. The elevated INR resolved after holding warfarin for two days, then reducing the dose by 11%. It is thought that the beta-glucan constituent of maitake mushroom might cause warfarin dissociation from proteins, resulting in increased free warfarin levels and increased warfarin effects.
Cordyceps
Anticoagulant/Antiplatelet Drugs
Theoretically, cordyceps may increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
In vitro and animal research suggests that cordyceps extract inhibits platelet aggregation and function. However, this interaction has not been reported in humans.
Immunosuppressants
Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.
Testosterone
Theoretically, concurrent use of cordyceps and testosterone might have additive effects.
Animal research suggests that cordyceps can increase testosterone levels. The clinical significance of this finding is unclear.
Himematsutake
Antidiabetes Drugs
Theoretically, taking agaricus mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
In one clinical study in patients with type 2 diabetes who are stabilized on conventional oral hypoglycemic agents, 3 of 29 patients taking an agaricus mushroom extract 500 mg three times daily for 12 weeks reported hypoglycemia, compared to one of 29 patients in the placebo group.
