Andrographis is generally well tolerated in the short term, though long-term safety and product quality aren't well established. The most common side effects are abdominal discomfort, altered taste, diarrhea, dizziness, fatigue, headache, nausea, vomiting, rash, itching, and hives — and they're more likely at higher doses or beyond 14 days of use.

Severe allergic reactions, including anaphylaxis, are rare but possible. Skin reactions — including Stevens-Johnson syndrome and other serious rashes — have been reported.

Avoid andrographis during pregnancy because of theoretical and animal-study concerns about reproductive effects. Also avoid it while breastfeeding because there isn't enough safety information.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
