HelloPharmacist Scorecard of Phenbuterol by MuscleMeds
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
Phenbuterol contains five active ingredients. Caffeine anhydrous is a stimulant that increases alertness and energy.
Yerba mate leaf extract is a plant source that also contains caffeine. Bitter orange (Advantra Z) contains synephrine and other compounds with stimulant properties.
Indian snakeroot extract (Rauwolfia serpentina) contains alkaloids like reserpine and yohimbine that affect heart rate and blood pressure. Grains of Paradise seed extract is included as a thermogenic agent.
The product has no inactive ingredients listed.
Does it work?
Strong evidence
The evidence for this product's active ingredients is mixed. Caffeine has established effectiveness for neonatal apnea and postoperative headache, is likely effective for mental alertness and athletic performance, and is possibly effective for bronchopulmonary dysplasia — but those clinical uses don't reflect what a healthy adult takes this product for.
For yerba mate, the data we hold shows insufficient evidence to rate it for athletic performance, arrhythmia, osteoporosis, chronic fatigue, headache, or cognitive function. Bitter orange, Indian snakeroot, and Grains of Paradise all show insufficient evidence to rate for their claimed uses.
The product's overall muscle-building and fat-burning claims are not covered in our effectiveness data.
How safe is it?
Well-documented data
Caffeine in moderate doses is generally well tolerated, though high amounts carry serious risks. Common side effects include anxiety, insomnia, tremors, nausea, restlessness, and headache.
Rarely, stroke has been reported. Yerba mate has similar caffeine-related risks and can cause tachycardia and nervousness; long-term heavy use raises additional concerns.
Bitter orange can raise blood pressure and heart rate, especially in combination with caffeine, and may cause hypertension and tachycardia as common effects. Indian snakeroot is potentially unsafe at higher doses and frequently causes sedation, low blood pressure, slow heart rate, weight gain, and nightmares; serious adverse effects (rare) include heart attack, atrial fibrillation, and depression.
Grains of Paradise safety is not well studied in supplement amounts. Pregnancy: Caffeine is possibly unsafe at high doses; yerba mate and bitter orange are possibly unsafe; Indian snakeroot should be avoided.
Breastfeeding: Small amounts of caffeine pass into milk and are usually acceptable with moderation; yerba mate is possibly unsafe; bitter orange and Indian snakeroot should be avoided.

Major interaction found
Check with your pharmacist or doctor immediately if you take ephedrine (Major risk of life-threatening stimulant effects), MAOIs (Major hypertensive crisis risk), midazolam or other sedatives, beta-blockers, digoxin, levodopa, antipsychotics, blood thinners, blood pressure medications, fluoroquinolone antibiotics, seizure drugs like phenobarbital or carbamazepine, clozapine, cimetidine, dipyridamole, dextromethorphan, blood sugar medications, or any CYP3A4 substrate (a large group including many statins and blood pressure drugs). No interactions are documented for Grains of Paradise.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with clinical evidence supporting its stated purpose. Major medication interactions have been identified, and safety information is well characterized.
This is a high-stimulant, multi-ingredient product with serious interactions — especially if you take heart medications, blood pressure drugs, seizure medicines, antipsychotics, antidepressants, or any sedative. Because Indian snakeroot can cause depression and significant blood pressure changes, it's not suitable for routine supplementation without medical oversight.
Talk to your pharmacist or doctor before starting, and bring a list of all your medications to check against our interaction database.

Phenbuterol by MuscleMeds Drug Interactions
HelloPharmacist Interaction Report
Phenbuterol by MuscleMeds contains five active ingredients, and the product carries significant interaction concerns.
The most serious risk comes from caffeine anhydrous and yerba mate (which also contains caffeine) combined with ephedrine — a Major interaction that can trigger life-threatening stimulant effects including dangerous blood pressure spikes and heart attacks.
Read the full breakdown — every affected drug type, severity by severity
Bitter orange (Advantra Z) adds Major risks with MAOIs (blood pressure crisis) and midazolam (sedative overdose), plus Moderate concerns with many other drugs: CYP3A4 substrates (a large drug class including some cholesterol and blood pressure medications), blood sugar medications, other stimulants, dextromethorphan (cough suppressant), and caffeine itself (which amplifies heart rate and blood pressure effects). The caffeine ingredients also interact Moderately with sedatives like pentobarbital, blood thinners and vasodilators like dipyridamole, antipsychotics like clozapine, stomach acid blockers like cimetidine, fluoroquinolone antibiotics, and seizure medications like phenobarbital and carbamazepine.
Indian snakeroot extract carries four Major interactions: beta-blockers (risk of slow heart rate and low blood pressure), antipsychotics, levodopa (Parkinson's medication), and digoxin (heart medication). It adds Moderate risks with sedatives, blood pressure medications, certain enzyme substrates, and blood thinners.
Altogether, these interactions span 1,332 individual medications. We could not check Grains of Paradise for interactions — no data is on file for it.
