Why this applies to Bisoprolol
Bisoprolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Bitter Orange interacts with.
In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with bitter orange and your bisoprolol. Lab studies suggest one compound in bitter orange (octopamine) might weakly slow down CYP2D6, an enzyme that helps clear bisoprolol from your body. In theory, that could let a bit more of the drug build up, which might mean a slower heart rate, lower blood pressure, dizziness, or tiredness. Keep in mind bisoprolol is mostly cleared other ways too, so this effect is likely small.
The bigger concern is the opposite tug-of-war: bitter orange is a stimulant that can raise heart rate and blood pressure, working against what your bisoprolol is trying to do. This is only test-tube evidence, not seen in real people, but I'd still run it by your pharmacist or doctor before using it.
