Aspirin, Carisoprodol, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Bitter Orange interacts with.

Theoretically, bitter orange might increase levels of drug metabolized by CYP2D6.
In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

Here's the wrinkle with this combo pill: the codeine part is a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Bitter orange contains a compound (octopamine) that may weakly block that same enzyme. If it does, less codeine gets converted, so you might get less pain relief than expected, not more.

That said, this is really a theoretical concern. It has only shown up in lab studies, never in real people, and the effect looks mild. The aspirin and carisoprodol pieces aren't the issue here. Bitter orange's bigger downside is its own stimulant effect on heart rate and blood pressure. If you're using both, just check in with your pharmacist or doctor first.
