Why this applies to Perphenazine

Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Bitter Orange interacts with.

Theoretically, bitter orange might increase levels of drug metabolized by CYP2D6.
In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP2D6 to break down perphenazine. Bitter orange contains a compound (octopamine) that may slightly slow that enzyme down. If it does, perphenazine could clear a little more slowly and build up, which in theory means stronger effects and more side effects like drowsiness, dizziness, dry mouth, a drop in blood pressure when you stand, or muscle stiffness and restlessness.

The good news is this is mostly theoretical. It comes from lab studies only and hasn't actually been seen in people, so the risk is rated minor. Bitter orange is also a stimulant that can nudge up heart rate and blood pressure, which is its own reason to be cautious. I'd just run it by your pharmacist or doctor before combining the two.

Bitter orange is a citrus fruit whose extracts contain synephrine, a mild stimulant often added to weight-loss and energy supplements. Evidence that it works for weight loss or performance is limited, and because it can raise heart rate and blood pressure, people with heart problems, high blood pressure, or who are pregnant should avoid it. Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
