CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Oxybutynin
Oxybutynin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

Here's the gist: your body uses an enzyme called CYP3A4 to help break down oxybutynin. Bitter orange can mildly block that enzyme, mostly in your gut. Oxybutynin is the active drug itself (not a prodrug it needs to switch on), so slowing its breakdown could let a bit more of it linger in your system.
In plain terms, that might mean stronger anticholinergic effects, things like dry mouth, blurry vision, constipation, drowsiness, or trouble passing urine. The evidence here is limited and comes from lower-quality studies, so this isn't a guaranteed problem, and the bigger concern with bitter orange may simply be its own stimulant effect on heart rate. Still, if you take oxybutynin daily, mention any bitter orange or weight-loss supplements to your pharmacist or doctor before combining them.

Bitter orange is a citrus fruit whose extracts contain synephrine, a mild stimulant often added to weight-loss and energy supplements. Evidence that it works for weight loss or performance is limited, and because it can raise heart rate and blood pressure, people with heart problems, high blood pressure, or who are pregnant should avoid it. Talk to your pharmacist or doctor before using it, especially if you take other stimulants.

What Is Oxybutynin Used For?
Brand names: Ditropan, Ditropan XL, Oxybutynin, Oxytrol, Anturol, Gelnique
Oxybutynin treats overactive bladder — including the sudden urge to urinate, leaking before you can reach the bathroom (urge incontinence), and going too frequently. Extended-release tablets (Oxybutynin Chloride) and immediate-release tablets and oral solution (Oxybutynin Chloride) are used for these symptoms. Oxytrol is approved for overactive bladder in men; Oxytrol for Women is approved for overactive bladder in women aged 18 and older.
Extended-release tablets (Oxybutynin Chloride) are also approved for children 6 years and older who have bladder overactivity caused by a neurological condition, such as spina bifida. Immediate-release tablets and oral solution (Oxybutynin Chloride) are indicated for bladder instability symptoms — including urgency, frequency, urinary leakage, and painful urination — in patients with neurogenic bladder conditions.
