Metoprolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Bitter Orange interacts with.

Theoretically, bitter orange might increase levels of drug metabolized by CYP2D6.
In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

Here's the gist with metoprolol and bitter orange. Your body clears metoprolol mainly through a liver enzyme called CYP2D6. Lab studies suggest one compound in bitter orange (octopamine) may weakly block that enzyme. In theory, that could let a little more metoprolol build up, which might nudge your heart rate or blood pressure lower than usual or add to tiredness and dizziness.

I want to be honest, though: this is only seen in test-tube research and hasn't been shown in real people, so it's a minor, theoretical concern. The bigger issue is that bitter orange is a stimulant that can raise heart rate and blood pressure, which works against what metoprolol is doing. If you're considering this supplement, please run it by your pharmacist or doctor first.

Bitter orange is a citrus fruit whose extracts contain synephrine, a mild stimulant often added to weight-loss and energy supplements. What Is Metoprolol Used For?

Brand names: Lopressor, Toprol XL

Metoprolol is used to treat several heart and blood pressure conditions. Metoprolol succinate extended-release tablets (including Toprol-XL) are approved for high blood pressure, angina pectoris (chest pain from the heart), and heart failure. Metoprolol tartrate tablets (including Lopressor) are approved for high blood pressure, angina, and — when used alongside intravenous metoprolol — to reduce the risk of cardiovascular death after a definite or suspected heart attack.

Metoprolol tartrate injection (intravenous form) is used in hemodynamically stable patients with a definite or suspected acute heart attack to reduce cardiovascular death, and is followed by oral metoprolol therapy. Lowering blood pressure with metoprolol reduces the risk of strokes and heart attacks.
