Acetaminophen, Propoxyphene falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Bitter Orange interacts with.

Theoretically, bitter orange might increase levels of drug metabolized by CYP2D6.
In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

This combination pairs a weight-loss stimulant with Darvocet, which contains propoxyphene (an opioid pain reliever) plus acetaminophen. The theory here is that a compound in bitter orange called octopamine may weakly block the CYP2D6 enzyme, one of the pathways your body uses to process propoxyphene. If that pathway slows down, a little more of the drug could linger, which in theory might nudge up opioid-type effects like drowsiness, dizziness, slowed breathing, or constipation.

Honestly though, this is a very minor, theoretical concern. It has only been seen in lab dishes, never confirmed in real people. There's a bigger point worth knowing: propoxyphene products like Darvocet were pulled from the U.S. market years ago due to heart-rhythm risks, and bitter orange itself can raise heart rate and blood pressure. Please check with your pharmacist or doctor before mixing these.
