Chlorpromazine falls under QT INTERVAL-PROLONGING DRUGS — the group Bitter Orange interacts with.
One case report suggests that taking bitter orange in combination with other stimulants such as caffeine might prolong the QT interval in some patients.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern in plain terms. Chlorpromazine can affect your heart's electrical timing, something doctors call QT prolongation. Bitter orange contains synephrine, a stimulant that can nudge up your heart rate and blood pressure. Put the two together and, at least in theory, you might add a little extra strain on that heart rhythm, which in rare cases could lead to an irregular heartbeat.
I want to be honest about how solid this is. The evidence here is weak, really just one case report involving bitter orange plus caffeine, so for most people the actual risk is small. Still, because chlorpromazine already touches heart rhythm, I'd skip stimulant-style weight-loss supplements with it and check with your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with bitter orange and chlorpromazine. Your body uses an enzyme called CYP2D6 to help break chlorpromazine down. One compound in bitter orange (octopamine) has been shown, in lab dish studies only, to weakly block that enzyme. In theory, slowing that cleanup could let chlorpromazine build up a bit, which might mean more of its usual effects like drowsiness, dizziness, dry mouth, or a drop in blood pressure when you stand up.
The honest truth: this has only been seen in test tubes, never in actual people, so it's rated minor. I'm a little more cautious here because bitter orange is also a stimulant that can nudge up your heart rate and blood pressure. If you're thinking about a weight-loss or energy product with bitter orange, run it past your pharmacist or doctor first.

What Is Chlorpromazine Used For?

Chlorpromazine is used to manage psychotic disorders and treat schizophrenia. It also controls nausea and vomiting, relieves restlessness and apprehension before surgery, and controls the manic phase of manic-depressive illness. Other uses include relief of intractable hiccups, treatment of acute intermittent porphyria, and use as an adjunct in tetanus treatment.
In children ages 1 to 12, it is used for severe behavioral problems marked by explosive or combative behavior, and for short-term treatment of hyperactive children with conduct disorders. The oral forms are also approved for the management of manifestations of psychotic disorders broadly, in addition to the uses shared with the injection form.
