Swanson Policosanol 10 mg contains three active ingredients: octacosanol, triacontanol, and hexacosanol—all naturally occurring waxy compounds known collectively as policosanol. Octacosanol is the main component of the policosanol blend and is responsible for most of the studied effects.
The product also contains two inactive ingredients (rice flour and gelatin) as fillers and capsule material.
Does it work?
Strong evidence
The evidence for these ingredients is mixed and limited. Triacontanol and hexacosanol are rated as likely effective for intermittent claudication (pain or cramping in the legs during walking).
Octacosanol, on the other hand, is rated as having insufficient reliable evidence for the conditions studied, including ALS, athletic performance, Parkinson disease, and high cholesterol (hyperlipidemia). For other uses like coronary heart disease and familial high cholesterol, evidence is also insufficient.

Well-documented data
All three active ingredients are generally well tolerated at studied doses, but long-term safety data are limited. Uncommon side effects include headache, mild dizziness, sleepiness, irritability, insomnia, itching, and skin rash.
Weight loss has been associated with policosanol use, though post-market data suggest it's uncommon at typical doses. Because there isn't enough safety information, the safety data advises against use during pregnancy and while breastfeeding.

All three active ingredients also interact with blood thinners and antiplatelet drugs (anticoagulants/antiplatelet agents), including warfarin. They may inhibit platelet aggregation, theoretically raising the risk of bruising and bleeding, though one clinical trial suggests the effect may not be clinically significant.
Additionally, all three can lower blood sugar (glucose) levels, so combining them with diabetes medications (antidiabetes drugs) might intensify that effect and increase the risk of low blood sugar (hypoglycemia). Octacosanol also has a documented interaction with levodopa/carbidopa (used for Parkinson disease), where it may worsen symptoms including involuntary movements.
Octacosanol, triacontanol, and hexacosanol may also enhance the blood pressure-lowering effects of nitroprusside, a potent IV blood pressure medication. Altogether, these interactions span 247 individual medications.

Policosanol
Anticoagulant/Antiplatelet Drugs
Theoretically, taking policosanol with other antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
Some clinical research shows that taking policosanol 10-50 mg daily for 7-15 days can inhibit platelet aggregation in healthy patients. Although, one clinical trial shows that taking policosanol 10 mg twice daily for 2 weeks prior to warfarin dosing does not affect warfarin pharmacokinetics or warfarin response. Furthermore, a study in patients undergoing percutaneous coronary intervention with a drug-eluting stent found that taking policosanol 40 mg plus clopidogrel and aspirin daily for 30 days modestly reduced the risk for minor bleeding events when compared with taking clopidogrel and aspirin alone. Until more is known, use with caution.
Antidiabetes Drugs
Theoretically, policosanol might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia.
Clinical research shows that policosanol can lower blood glucose levels.
Beta-Blockers
Concomitant use of policosanol with beta-blockers can cause additive blood pressure-lowering effects.
Clinical research shows that policosanol 5 mg daily can have additive blood pressure-lowering effects in patients with hypertension who are taking beta-blockers. Also, animal research shows that policosanol can increase the blood-pressure lowering effects of propranolol.
Nitroprusside (Nitropress)
Theoretically, concomitant use of policosanol with nitroprusside might cause additive blood pressure-lowering effects.
Animal research shows that taking policosanol along with nitroprusside can increase the blood pressure-lowering effects of nitroprusside.
Propranolol (Inderal)
Animal research shows that taking policosanol along with propranolol can increase the blood pressure-lowering effects of propranolol.
Warfarin (Coumadin)
Theoretically, taking policosanol with warfarin might increase the risk of bruising and bleeding.
Some clinical research shows that taking policosanol 10-50 mg daily for 7-15 days can inhibit platelet aggregation in healthy patients.
