Pharmax Probiotics contains 4 active ingredients: two strains of Lactobacillus acidophilus (CUL-60 and CUL-21), one strain of Bifidobacterium bifidum (CUL-20), and one strain of Bifidobacterium animalis subsp. lactis (CUL-34). These are live, beneficial bacteria meant to support gut and digestive health.
The capsules also contain cellulose and hypromellose as inactive ingredients (the capsule material and binders).

The evidence for these probiotics is mixed and depends on what you're hoping to use them for. Lactobacillus acidophilus shows possibly effective evidence for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis; it's possibly ineffective for atopic disease, and the evidence is insufficient for acute respiratory distress syndrome.
Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections, but evidence is insufficient for hay fever, Alzheimer disease, and generalized anxiety. Bifidobacterium animalis subsp. lactis is possibly effective for respiratory tract infections, IBS, and constipation.
In short: these strains may help with digestive and some respiratory complaints, but the evidence is modest and condition-dependent.
How safe is it?
Well-documented data
These probiotics are generally well tolerated by healthy adults and children. That said, the data come with caveats: there's potential concern that live probiotic organisms, in rare cases, might cause infection in people with weakened immune systems or certain serious illnesses, and a few isolated cases of serious infections (bacteremia, sepsis, endocarditis) have been tied to Lactobacillus acidophilus, though these are thought to arise from bacteria that translocated from elsewhere in the body rather than from the supplement itself.
More commonly, you might notice mild gastrointestinal side effects—bloating, flatulence, abdominal discomfort, or diarrhea—but these are uncommon. Rash and itching have been reported rarely in one trial, though it's unclear if the probiotics were responsible.

Suggested/Recommended/Usage/Directions
Recommended adult intake: Contents of one serrated blister pack strip: 1 tablet, 1 softgel, 1 capsule (brown), 1 capsule (off-white), all taken once daily with a meal, at least two to three hours before or after taking medications, or as professionally directed. Not to be taken immediately before bedtime.
Precautions

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium bifidum (CUL-20)
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
