Generally well tolerated in healthy adults and children, though live probiotics carry rare but serious risks in people who are critically ill, premature infants, or have weakened immune systems. The most common side effects are gastrointestinal: bloating, flatulence, abdominal discomfort, and diarrhea, especially when you first start or at higher doses.

L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
