The most common side effects are gastrointestinal — abdominal pain, cramping, nausea, vomiting, diarrhea, and constipation — though in one study these complaints happened at similar rates in people taking puncture vine and those on placebo.
Rare serious effects including liver and kidney injury, seizures, and chronic painful erection have been reported. Gelatin is generally well tolerated orally and may cause belching, bloating, upset stomach, or unpleasant taste in some people; in rare cases it can trigger allergic reactions including hives, swelling, wheezing, or anaphylaxis (particularly in people sensitized through gelatin in foods or vaccines).
Puncture vine safety during pregnancy is not established and the data advises against it because it may affect hormones. For breastfeeding, reliable safety information is lacking — it should be avoided.
For gelatin, pregnancy is rated likely safe to possibly safe, and lactation is rated likely safe, though supplement doses have little research behind them — talk with your doctor or pharmacist before using either during pregnancy or breastfeeding.

Antidiabetes Drugs
Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.
Antihypertensive Drugs
Theoretically, taking tribulus with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that tribulus can lower blood pressure by inhibiting angiotensin-converting enzyme (ACE). Tribulus has also demonstrated hypotensive effects in pre-hypertensive adults.
Lithium
Theoretically, tribulus might increase the levels and clinical effects of lithium.
Tribulus is thought to have diuretic properties. Due to these potential diuretic effects, tribulus might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
