Moderate evidence
The evidence supports L-glutamine for sickle cell disease, where it's rated effective. It's rated possibly effective for HIV/AIDS-related wasting, postoperative recovery, and critical illness from trauma — meaning there's some promise, but the evidence isn't yet as strong.
How safe is it?
Well-documented data
L-glutamine is generally well tolerated by healthy adults. The most common side effects are gastrointestinal: belching, bloating, constipation, diarrhea, flatulence, nausea, vomiting, and abdominal pain.
Headache, musculoskeletal pain, cough, and dizziness have also been reported. A few cases of mood changes (mania or hypomania) occurred in people with bipolar disorder taking high doses.
If you have kidney or liver disease, use it only under medical supervision. There isn't enough safety data to recommend it during pregnancy unless your doctor advises it, though it's rated likely safe in pregnancy.
For breastfeeding, there's limited data, so avoid supplement doses unless a clinician recommends it.
