Ultimate Probiotic Balance contains 15 active ingredients: multiple strains of live bacteria (Lactobacillus and Bifidobacterium species), Streptococcus thermophilus, and inulin. The evidence for this product's individual ingredients is mixed and mostly limited. Lactobacillus acidophilus and Bifidobacterium bifidum each show possibly effective evidence for irritable bowel syndrome (IBS); L. acidophilus is also possibly effective for antibiotic-associated diarrhea, Helicobacter pylori infection, and bacterial vaginosis.
Inulin is possibly effective for obesity, constipation, and diabetes. For most other conditions—including allergic rhinitis, asthma, anxiety, and cognitive decline—the evidence we hold is insufficient to rate these ingredients.
Streptococcus thermophilus's effectiveness hasn't been established in the data we hold for any condition listed.
How safe is it?

Overall, the individual bacterial strains are generally well tolerated in healthy adults and children. However, several safety cautions apply.
Since these are live organisms, there's a theoretical concern they could cause infection in people who are severely ill, immunocompromised, or have central venous catheters; rare cases of bacteremia and sepsis have been reported in critically ill patients and preterm infants. Gastrointestinal side effects—bloating, flatulence, cramping, diarrhea, and abdominal discomfort—are common, especially with inulin at higher doses (over 30 grams).
One rare but serious concern: anaphylaxis has occurred after inulin consumption. Because safety data are limited, especially for supplement doses, check with your doctor before use, particularly if you're pregnant, breastfeeding, or have a weakened immune system.

Theoretically, taking Lactobacillus gasseri with antibiotic drugs might decrease the effectiveness of L. gasseri.
Lactobacillus gasseri preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. gasseri preparations by at least two hours.
Bifidobacterium breve Bb-03
Antibiotic Drugs
Theoretically, taking Bifidobacterium breve with antibiotic drugs might decrease the effectiveness of B. breve.
Since B. breve preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. breve preparations by at least 2 hours.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.

Inulin
Antidiabetes Drugs
Theoretically, inulin might increase the risk of hypoglycemia with antidiabetes drugs.
Some clinical research shows that inulin improves glycemic control in patients with diabetes; however, it is unclear if it has hypoglycemic effects.
