Ultra Mega Duo Dophilus is a nine-ingredient probiotic capsule. The active ingredients are live bacterial strains: Bifidobacterium longum Bl-05, Bifidobacterium bifidum Bb-02, Bifidobacterium lactis Bl-04, Lactobacillus acidophilus La-14, Lactobacilli plantarum Lp-115, Lactobacilli rhamnosus Lr-32, Lactobacilli bulgaricus Lb-64, and Lactobacilli salivarius Ls-33.
The product also contains FOS, a carbohydrate that may help feed beneficial bacteria in your gut. The remaining ingredients—vegetarian capsules, cellulose, magnesium, stearic acid, vegetable stearin, and silica—are inactive excipients that make up the capsule and ensure the product holds together.
Does it work?
Moderate evidence
The evidence for this product's effectiveness is mixed and varies by strain. Lactobacillus acidophilus and Bifidobacterium bifidum both show possibly effective evidence for irritable bowel syndrome and respiratory tract infections, and L. acidophilus also has possibly effective evidence for antibiotic-associated diarrhea, Helicobacter pylori infection, and bacterial vaginosis.
However, for many other conditions—cognitive decline, hay fever, asthma, athletic performance, and common cold—the evidence is insufficient to rate these strains. FOS and the unchecked bacterial strains have no effectiveness data on file in our database.
How safe is it?
Well-documented data
These probiotics are generally well tolerated in healthy people, though caution is advised for anyone who is seriously ill or immune-compromised, as rare cases of bacterial infection (bacteremia or sepsis) have occurred in critically ill patients and preterm infants. Gastrointestinal side effects are uncommon but have been reported and include flatulence, bloating, abdominal discomfort or pain, constipation, and rarely diarrhea or vomiting.
A single case of dry cough was reported in a trial. Pregnancy and lactation safety data are limited; Bifidobacterium bifidum has no pregnancy/lactation rating on file, and L. acidophilus and L. bulgaricus have none either.
Bifidobacterium longum is rated possibly safe in pregnancy and lactation, but consult your doctor before use.

Bifidobacterium longum Bl-05
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
Bifidobacterium bifidum Bb-02
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
