How safe is it?
Well-documented data
Nattokinase is generally well tolerated in the short term, but because of its blood-thinning effects, it should be used with caution if you have a bleeding disorder or any bleeding risk. The most notable adverse effect is a rare risk of hemorrhage (bleeding), including a documented case of brain bleeding when nattokinase was combined with aspirin.
Additionally, nattokinase can trigger hypersensitivity reactions in people allergic to natto or its components — symptoms like throat and larynx swelling can occur within 2 hours of taking it. The safety data advises against using nattokinase during pregnancy and breastfeeding.

Pine bark extract
Anticoagulant/Antiplatelet Drugs
Theoretically, maritime pine bark extract might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Clinical research suggests that maritime pine bark extract inhibits platelet aggregation. However, the clinical significance of this effect is unclear.
Antidiabetes Drugs
Theoretically, maritime pine bark extract might increase the risk of hypoglycemia when used with antidiabetes drugs.
One clinical study shows that maritime pine bark extract decreases blood sugar in patients with diabetes being treated with antidiabetes agents. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Immunosuppressants
Theoretically, maritime pine bark extract might decrease the effectiveness of immunosuppressant therapy.
In vitro and animal research suggests that maritime pine bark extract has immunostimulant activity. This effect has not been reported in humans.
Nattokinase
Anticoagulant/Antiplatelet Drugs
Nattokinase might increase the risk of bleeding when used with anticoagulant/antiplatelet drugs.
Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
Antihypertensive Drugs
