Sodium chloride breaks apart in water into two ions: sodium and chloride. Sodium is the body's main electrolyte outside cells — it controls how water moves between body compartments and is essential for maintaining fluid balance. Chloride supports the exchange of oxygen and carbon dioxide in the red blood cells. The kidneys carefully regulate how much sodium and chloride the body retains or excretes to keep everything in balance. When IV sodium chloride is given, it directly replaces what the body is missing and restores normal fluid and electrolyte levels.

Side effects depend on how sodium chloride is being used — most of the known reactions are associated with IV administration.
- Fever or febrile response

- Vein inflammation (phlebitis) or blood clot near the infusion site

⚠️Warnings & precautions▾
- Fluid overload: IV sodium chloride can cause too much fluid to build up in the body, leading to lung or tissue swelling. This risk is higher in patients with heart, lung, or kidney problems.
- Low sodium (hyponatremia): IV sodium chloride can paradoxically cause sodium levels to drop too low. This can lead to headache, nausea, seizures, brain swelling, coma, or death — especially in children, pre-menopausal women, and patients with certain illnesses.
- High sodium (hypernatremia): too much sodium chloride can push sodium levels dangerously high, causing swelling and serious neurological harm.
- Correcting sodium imbalances too quickly is dangerous: rapid correction of either low or high sodium can cause osmotic demyelination syndrome (ODS) — nerve damage that can result in seizures or brain injury.
- Allergic/infusion reactions: reactions including low blood pressure, fever, chills, hives, and rash have been reported with 0.9% Sodium Chloride Injection. Stop the infusion immediately if these occur.
- High-concentration injections (3% and 5%): must not be used when sodium levels are already normal or elevated.
🔀What it interacts with▾
Sodium chloride IV solutions can interact with certain medications in ways that affect fluid and electrolyte balance:
- Corticosteroids (e.g., prednisone) and corticotropin: increase sodium and fluid retention, raising the risk of fluid overload and high sodium levels
- Diuretics (water pills): can increase the risk of low sodium (hyponatremia) when used alongside IV sodium chloride
- Certain antiepileptic drugs: may raise the risk of hyponatremia
- Certain psychotropic (psychiatric) medications: may also increase hyponatremia risk
- Drugs mixed into sodium chloride solutions: not all medications are compatible with sodium chloride — always check with a pharmacist before mixing

📋Before taking it▾
- Tell your doctor if you have heart failure, high blood pressure, kidney disease, liver disease (including cirrhosis), or pre-eclampsia — these increase the risk of fluid and sodium imbalances with IV use
- Avoid high-concentration (3% or 5%) sodium chloride injections if your sodium levels are normal or high
- Older adults are more likely to have reduced kidney function and may need lower doses and closer monitoring
- Children require careful electrolyte monitoring, as they may have limited ability to regulate fluids and salts
- Pregnant women should only use sodium chloride IV solutions when clearly necessary — animal safety studies have not been conducted
- Breastfeeding mothers should use IV sodium chloride with caution; it is not known whether it passes into breast milk
- Bacteriostatic Sodium Chloride contains benzyl alcohol, which may be incompatible with certain drugs — verify compatibility before use
- People taking diuretics, antiepileptics, or psychiatric medications should be monitored closely for sodium imbalance
🚑If you take too much▾
Too much sodium chloride given intravenously can cause serious problems. Excess 0.9% Sodium Chloride Injection can lead to high sodium levels (hypernatremia), while excess 0.45% Sodium Chloride Injection can cause either high or low sodium — both can lead to seizures, coma, brain swelling, or death. Fluid overload, which can cause fluid buildup in the lungs or body tissues, is also a risk. If an overdose is suspected, the infusion should be stopped and treated promptly by a medical team, with monitoring of fluid balance, electrolyte levels, and acid-base status.
