Sertraline falls under SEROTONERGIC DRUGS — the group Acetyl-l-carnitine interacts with.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: sertraline (Zoloft) works by raising serotonin in your brain, and acetyl-L-carnitine may nudge serotonin up too, at least in animal studies. Put the two together and there's a theoretical chance of having a bit too much serotonin activity. In rare cases that can lead to something called serotonin syndrome, with symptoms like restlessness, sweating, shivering, a racing heart, muscle twitching, or feeling agitated or confused.
I want to keep this in perspective. This concern comes from lab and animal research, not from real-world reports in people, so for most folks the risk is low. Still, since both push in the same direction, it's worth a quick chat with your pharmacist or doctor before adding the supplement, and don't change your sertraline on your own.

Sertraline treats a range of mental health conditions. All formulations are approved for major depressive disorder (MDD) in adults and obsessive-compulsive disorder (OCD). The oral solution (including Zoloft oral solution) and several tablet formulations are also approved for panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).
For OCD specifically, the capsule formulations and some tablet products are approved for use in adults and children aged 6 and older. Sertraline hydrochloride tablets (plain, not film-coated) are approved for OCD in pediatric patients as well. However, sertraline is not approved for major depressive disorder in children — only in adults.
