Here's the honest picture with Hyzaar (losartan plus hydrochlorothiazide). The concern on file is that American elder might block a liver enzyme called CYP3A4, which in theory could let certain drugs build up. But this one is mostly lab-dish science, not real people, so I wouldn't lose sleep over it.

For losartan specifically, it's actually a partial prodrug. Part of it gets converted by liver enzymes into a stronger active form, so slowing those enzymes could in theory slightly weaken its blood-pressure effect, not boost it. The water-pill part (hydrochlorothiazide) isn't affected this way. Practically, eating properly cooked elderberries in food amounts is unlikely to matter. If you're using concentrated elderberry supplements daily, just mention it to your pharmacist or doctor so they can keep an eye on your blood pressure.
