American Spikenard and Hydrochlorothiazide, Lisinopril

Here is the interaction we found between these two, with the evidence behind it.

Minor

Diuretic Drugs

Why this applies to Hydrochlorothiazide, Lisinopril

Hydrochlorothiazide, Lisinopril falls under Diuretic Drugs — the group American Spikenard interacts with.

American spikenard is purported to have diaphoretic properties. Diaphoretic substances can increase sweating and fluid loss from the body, which may enhance the effects of diuretics. This interaction could potentially lead to excessive fluid loss and electrolyte imbalance, especially in individuals taking diuretic medications. Healthcare providers need to monitor patients closely for signs of dehydration or electrolyte disturbances if they are using both Aralia racemosa and diuretic drugs concurrently.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your blood pressure pill is really two medicines in one. The hydrochlorothiazide part is a water pill that already makes you pee out extra fluid and salt. American spikenard has a traditional reputation for making you sweat (what older books call a 'diaphoretic'). The worry here is simple: a water pill plus something that makes you sweat could add up to losing a little too much fluid.

If that happened, you might feel lightheaded, dizzy, very thirsty, or weak, and your potassium or sodium could drift off. Honestly, this is rated minor and rests on weak, theoretical evidence, so for most people it is not a big deal. Still, stay well hydrated, and check with your pharmacist or doctor before combining them, especially in hot weather.

From our pharmacist’s overview on the Hydrochlorothiazide, Lisinopril page

“I would not add a supplement casually alongside hydrochlorothiazide and lisinopril (Zestoretic, Prinzide). This combination lowers blood pressure with a water pill plus an ACE inhibitor, and most interacting ingredients either drag pressure down too far or push back against the drug, while smaller groups strain the kidneys, drain potas…”

About Lisinopril and Hydrochlorothiazide

What Is Lisinopril and Hydrochlorothiazide Used For?

Brand names: Zestoretic, Prinzide

Lisinopril and hydrochlorothiazide tablets — sold under the brand name Zestoretic and as the generic combination — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events, including strokes and heart attacks.

This combination is not intended as a first-line starting treatment. It is typically used when a single blood pressure medication alone has not been enough to reach target blood pressure goals.
