Here's the simple version: metolazone is a strong water pill that already pulls extra salt and water out of your body through your urine. American spikenard is traditionally thought to be a diaphoretic, meaning it may make you sweat a bit more. If you stack the two, the worry is that you lose even more fluid than intended.
In real terms, that could mean feeling lightheaded, dizzy, weak, or extra thirsty, or developing low potassium or sodium (which can cause muscle cramps or a fast heartbeat). I'll be honest though: this is rated a minor interaction with weak, mostly theoretical evidence, so for most people it's a low concern. Still, since metolazone is potent, please run it by your pharmacist or doctor before combining them, and watch for those dehydration signs.

What Is Metolazone Used For?
Brand names: Diulo, Zaroxolyn, Mykrox
Metolazone tablets help your body get rid of extra salt and water. They are used for swelling (edema) caused by heart failure and by kidney diseases, including nephrotic syndrome and reduced kidney function.
They are also used to treat high blood pressure, either alone or with other blood pressure medicines from a different class. Mykrox tablets, a faster-acting form of metolazone, are meant for new patients with mild to moderate high blood pressure.
