Why this applies to Iloprost

Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Ash interacts with.

Theoretically, concomitant use of ash seed with antihypertensive drugs might have additive blood pressure lowering effects. Animal research shows that ash seed extract can decrease blood pressure. This effect has not been evaluated in humans. Until more is known, use cautiously in patients taking antihypertensive drugs.
Some antihypertensive drugs include nifedipine (Adalat, Procardia), verapamil (Calan, Isoptin, Verelan), diltiazem (Cardizem), isradipine (DynaCirc), felodipine (Plendil), amlodipine (Norvasc), and others.

Here's the simple version. Iloprost already works by relaxing and widening your blood vessels to ease the pressure in your lungs. Ash seed, at least in animal studies, also seems to nudge blood pressure downward. Stack the two together and the worry is that your blood pressure could dip a little too low. With Iloprost, that might show up as more lightheadedness, dizziness, flushing, a headache, or feeling faint when you stand up.

I'll be honest with you: this is theoretical. It comes from lab and animal work, not from people, so we don't truly know how big a deal it is. Iloprost is not a prodrug, so this isn't about how your body activates it, it's just two things lowering pressure at once. Please check with your pharmacist or doctor before combining them rather than guessing.

Aurlumyn (iloprost injection) is FDA-approved to treat severe frostbite in adults. Specifically, it is used to reduce the risk of losing fingers or toes (digit amputation) after serious frostbite injury. The clinical evidence supporting this use comes from young, healthy adults who suffered frostbite at high altitudes.
