Bepridil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.

Here's the gist for Bepridil: your liver clears it using an enzyme called CYP3A4. In lab dishes, astaxanthin has been shown to speed that enzyme up. If the same thing happened in people, your body might break down bepridil a little faster, which could lower its blood levels and make it work less well for controlling your chest pain (angina).

I want to be honest about how soft this evidence is. This was only seen in test-tube and cell studies, never confirmed in real patients, so it's mostly theoretical. Still, bepridil is a heart drug that needs steady levels, so it's not one I'd guess about. Please loop in your pharmacist or doctor before adding astaxanthin so they can keep an eye on things.
