CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with thiotepa: it's actually a prodrug. Your liver uses an enzyme called CYP3A4 to turn it into its active, cancer-fighting form (called TEPA). So anything that speeds up CYP3A4 could shift how much active drug you end up with. In lab dish studies, astaxanthin appears to rev up CYP3A4. In theory, that could change the balance of active drug, which matters because thiotepa's effectiveness and its toxicity (low blood counts, nausea, mouth sores) sit on a fine line.
That said, this is only seen in test tubes, never confirmed in people, so I don't want to alarm you. Still, thiotepa is serious chemotherapy. Please run any supplement, including astaxanthin, by your oncology team before adding it.

In vitro research shows that astaxanthin induces cytochrome CYP2B6 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with thiotepa: it's actually a prodrug. Your liver uses an enzyme called CYP2B6 to convert thiotepa into TEPA, the form that does much of the cancer-fighting work. So anything that speeds that enzyme up could change how the drug behaves.
In lab dishes, astaxanthin appears to rev up CYP2B6. In theory, that could shift how thiotepa is processed and how much active drug your body makes, which matters a lot for a chemotherapy this powerful. I'll be honest though, this is only test-tube data and has never been shown in real people, so it's far from proven. Still, because thiotepa is serious cancer treatment, please don't add astaxanthin without first clearing it with your oncology team.

Astaxanthin is a reddish carotenoid pigment with strong antioxidant activity in the lab, and it is widely promoted for skin, eye, heart, and exercise benefits. Early human studies are promising for a few uses, but the overall evidence is still limited and not strong enough to make firm claims. About Thiotepa Injection
What Is Thiotepa Injection Used For?
Brand names: Thioplex, Tepadina
Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.
Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
