Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Bael interacts with.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others. Use bael cautiously or avoid in patients taking these drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
In this combo pill, the piece that bael could touch is the caffeine. Your body clears caffeine using a liver enzyme called CYP1A2, and in lab (test tube) studies, bael appeared to slow that enzyme down. If that held true in people, caffeine might hang around a little longer, so in theory you could feel jittery, have a racing heart, or notice trouble sleeping.
Here's my honest take: this is only theoretical and based on lab data, not real-world reports, so I wouldn't lose sleep over it. The aspirin and codeine parts aren't really affected by this pathway. Culinary amounts of bael fruit are very unlikely to matter; concentrated supplements are the bigger question. Still, please run it by your pharmacist or doctor before pairing them.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use bael cautiously or avoid in patients taking these drugs.

Here's the honest picture with this combo pill (aspirin, caffeine, and codeine). In lab dishes, bael and two of its compounds slowed down a liver enzyme called CYP3A4. That enzyme plays a small role in clearing caffeine and a minor part in handling codeine. This has never actually been seen in real people, so we're talking about a theoretical concern based only on test-tube work.
If anything did happen, you might feel a touch more wired or jittery from the caffeine, or slightly more drowsiness from the codeine. Codeine is also a prodrug, meaning your body turns it into morphine using a different enzyme (CYP2D6), so bael's CYP3A4 effect isn't likely to change your pain relief much. Food amounts of bael almost certainly don't matter. Still, if you take concentrated bael supplements, mention it to your pharmacist or doctor.
