Sofosbuvir, Velpatasvir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bael interacts with.

Bael and its constituents marmelosin and marmesinin inhibited cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, bael might increase levels of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use bael cautiously or avoid in patients taking these drugs.

Here's the short version: in lab dishes, bael (and its compounds marmelosin and marmesinin) slowed down an enzyme called CYP3A4. Velpatasvir, one half of your Epclusa, is partly handled by that enzyme. So in theory, blocking it could let velpatasvir build up a bit higher than usual. The sofosbuvir part isn't really driven by CYP3A4, so that piece is less of a concern.

I want to be honest about the strength here: this is only a test-tube finding and has never been seen in real people, so I wouldn't lose sleep over it. Still, since finishing a full course of Epclusa cleanly matters for clearing hepatitis C, it's worth mentioning your bael use to your pharmacist or doctor before mixing them. Food amounts of the fruit are far less likely to matter than concentrated supplement doses.

What Is Sofosbuvir and Velpatasvir Used For?

Brand name: Epclusa

Sofosbuvir and velpatasvir treats chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 — covering all the major strains. It is approved for adults and children 3 years of age and older, whether or not they have cirrhosis (liver scarring).

It is used on its own in patients without cirrhosis or with compensated cirrhosis (where the liver is scarred but still functioning). In patients with decompensated cirrhosis (more advanced liver disease where the liver is no longer coping), it is used in combination with another medicine called ribavirin. Epclusa is a brand that is also available as oral pellets for younger children.
