Here's the concern with Epclusa (sofosbuvir/velpatasvir) and St. John's wort. This herb revs up a drug-clearing system in your body called CYP3A4, and it also pushes on a transporter called P-glycoprotein. The result is that your body clears Epclusa faster, so the blood levels drop below where they need to be.

That matters a lot here. You take Epclusa for a set course to cure hepatitis C, and if its levels fall too low, the virus may not be fully knocked out. That can mean treatment failure and even a harder-to-treat virus down the road. In fact, St. John's wort is generally listed as something to avoid during this therapy. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first so your cure isn't put at risk.

St. John's wort decreases the levels and clinical effects of P-glycoprotein substrates.
St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.

Here's the concern with pairing St. John's wort and Epclusa (sofosbuvir/velpatasvir). This herb speeds up a transporter called P-glycoprotein, which acts like a gatekeeper in your gut. When that gate works overtime, less of your hepatitis C medicine gets absorbed, so the levels in your blood drop. Velpatasvir is especially sensitive to this.

Lower drug levels can mean the medication doesn't fully clear the virus, and that's a real problem because finishing a full, effective course is how this treatment cures hepatitis C. This combination is actually flagged as one to avoid. Please don't stop either one on your own, but do talk with your pharmacist or doctor before starting St. John's wort.

Clinical research shows that taking St. John's wort 325 mg three times daily for 14 days along with bupropion, a CYP2B6 substrate, reduces the area under the concentration-time curve by approximately 14% and increases the clearance of bupropion by approximately 20%.

Here's the short version: St. John's wort revs up certain drug-clearing pathways in your body, which can lower the levels of medicines that pass through them. With Epclusa (sofosbuvir/velpatasvir), this is a real concern. St. John's wort is a strong inducer of the systems (especially the transporter P-glycoprotein and related enzymes) that move and clear velpatasvir, so it can drop your drug levels and let the hepatitis C virus keep replicating.

That matters because the whole point of your treatment course is to fully clear the virus. Weaker drug levels can mean treatment failure, and that's why the official prescribing information lists St. John's wort as something to avoid with Epclusa. Please don't stop or change anything on your own, just check with your pharmacist or doctor before combining these.

What Is Sofosbuvir and Velpatasvir Used For?

Brand name: Epclusa

Sofosbuvir and velpatasvir treats chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 — covering all the major strains. It is approved for adults and children 3 years of age and older, whether or not they have cirrhosis (liver scarring).

It is used on its own in patients without cirrhosis or with compensated cirrhosis (where the liver is scarred but still functioning). In patients with decompensated cirrhosis (more advanced liver disease where the liver is no longer coping), it is used in combination with another medicine called ribavirin. Epclusa is a brand that is also available as oral pellets for younger children.
