Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beet interacts with.

Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.

Here's the idea behind this one. Your body uses an enzyme called CYP3A4 to help break down irinotecan (Onivyde). Lab studies hint that betanin, the red pigment in beets, might slow that enzyme down, much like a known blocker does. If that happened in real life, more irinotecan could linger in your system, which could mean stronger side effects like severe diarrhea, low blood counts, nausea, and fatigue.

The reassuring part: this is only a test-tube finding, not something proven in people, so it's theoretical. Everyday beets in your meals are very unlikely to matter. Concentrated beetroot supplements are the bigger question mark. Since irinotecan is potent chemo, please run any beet supplement past your oncology team or pharmacist first.
