This combination product pairs hyoscyamine with phenobarbital, and it's the phenobarbital piece that matters here. Phenobarbital is a barbiturate, so it already calms the nervous system and can leave you feeling drowsy, foggy, or sluggish. The thought is that berberine may add a little sedation of its own on top of that, so the two together could leave you sleepier or less alert than you'd expect.

I'll be honest with you: this concern comes from animal studies only, so it's more of a theoretical heads-up than a proven problem in people. Still, since phenobarbital is potent, it's worth mentioning to your pharmacist or doctor before adding berberine, especially if you notice extra grogginess or trouble driving.
