There is some concern that electroencephalogram (EEG) biofeedback might lower the seizure threshold in some patients. Theoretically, combining EEG biofeedback with seizure threshold-lowering drugs might increase the risk of seizure.
Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the short version: this isn't a chemical clash. Biofeedback doesn't change how your body absorbs, breaks down, or clears methotrexate at all. The only flag on file is a theoretical one, and it's specifically about EEG (brainwave) biofeedback, which in rare cases might nudge the seizure threshold a little lower. Methotrexate is listed because it can, uncommonly, contribute to lowering that threshold too.

In real life this is a very low-level concern, based on weak evidence rather than reports of people actually having problems. The bigger things to watch with methotrexate are mouth sores, fatigue, nausea, and blood count and liver changes, none of which biofeedback affects. If you have a seizure history or are doing EEG-based biofeedback, just mention it to your doctor first. Otherwise, this combination is generally fine.
