Here's the connection worth knowing about. Methyldopa is one of the blood pressure medicines that can, in some people, stress the liver. It's actually known for occasionally causing liver inflammation or abnormal liver enzyme tests. Black cohosh has its own rare reports of liver trouble, including hepatitis and very uncommonly liver failure. The worry is simply that stacking two things that each can bother the liver might add up.

I want to be honest about the strength here: this is mostly theoretical and based on scattered case reports, not strong studies, so don't panic. Still, if you take methyldopa and are thinking about black cohosh, please check with your pharmacist or doctor first. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and report those right away.
