Why this applies to Fluphenazine

Fluphenazine falls under PHENOTHIAZINES — the group Black Currant interacts with.

Theoretically, black currant seed oil might increase the risk of seizure in patients receiving phenothiazines.
Black currant seed oil contains gamma-linolenic acid (GLA). There is some concern that taking supplements containing GLA might cause seizures, or lower the seizure threshold, when taken with phenothiazines, although there is no evidence that black currant seed oil causes seizures. In one report, three patients with schizophrenia who had received phenothiazines developed EEG changes suggestive of temporal lobe epilepsy after starting treatment with GLA, although none experienced an actual seizure. In another report, two patients with schizophrenia who were stabilized on phenothiazines developed seizures when evening primrose 4 grams daily, which contains GLA, was added. One of these patients had a prior history of seizures.

Here's the gist with fluphenazine (Prolixin) and black currant. Fluphenazine is an antipsychotic that can already lower the seizure threshold a little on its own. The concern is with black currant seed oil, which supplies an omega-6 fatty acid called GLA. There's a thought that adding GLA on top of a phenothiazine like fluphenazine could nudge that seizure threshold lower, making a seizure slightly more likely.

I want to be honest about how shaky this is. It's theoretical, drawn from a couple of old case reports with primrose oil and some EEG changes, not from black currant itself. Eating the berries is not the issue here; concentrated seed oil capsules are. Before adding any GLA supplement, check with your pharmacist or doctor first.

What Is Fluphenazine Used For?

Brand names: Prolixin, Permitil, Fluphenazine

Fluphenazine hydrochloride tablets and oral solution are indicated for the management of psychotic disorders. Fluphenazine Hydrochloride Injection is also used for psychotic disorders and can bridge patients to oral therapy.

Fluphenazine Decanoate Injection is a long-acting form specifically intended for patients who need extended antipsychotic treatment — for example, those with chronic schizophrenia who benefit from a single injection that can last up to four weeks or longer. It is not approved for behavioral problems related to intellectual disability, and it is not approved for dementia-related psychosis in elderly patients.
