Why this applies to Perampanel
Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Black Hoof Mushroom interacts with.
In vitro, polysaccharides from black hoof mushroom grown in culture inhibit CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Perampanel (Fycompa). Your liver uses an enzyme called CYP1A2 as one route to clear this seizure medicine. In test-tube studies, compounds in Black Hoof Mushroom seem to slow that enzyme down. If that happened in your body, perampanel could build up a bit and you might notice stronger side effects like dizziness, sleepiness, trouble with balance, or mood and behavior changes.
I want to be honest about the strength of this, though. This is only a lab finding, and it has not been seen in real people, so it stays theoretical. I wouldn't lose sleep over it, but since perampanel already causes drowsiness, please check with your pharmacist or doctor before pairing the two so they can keep an eye on how you feel.

Perampanel treats partial-onset seizures (with or without secondarily generalized seizures) in people with epilepsy who are 4 years of age and older.
It is also used as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures in people with epilepsy who are 12 years of age and older. This applies to Fycompa and to perampanel tablets and oral suspension.
