Black Psyllium and Bendroflumethiazide

Here is the interaction we found between these two, with the evidence behind it.

Minor

ORAL DRUGS

Why this applies to Bendroflumethiazide

Bendroflumethiazide falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: black psyllium is a soluble fiber that turns into a thick gel when it hits water in your gut. That gel can sometimes grab onto a pill and slow or reduce how much of it your body soaks up. Bendroflumethiazide is a water pill (thiazide diuretic) you take by mouth to lower blood pressure and clear extra fluid, so in theory the fiber could blunt a bit of its absorption and make it work a little less well.

The good news is this is rated minor and largely theoretical, with no strong proof it matters for this specific drug. To stay safe, just space them out: take your bendroflumethiazide first, then your psyllium about 30 to 60 minutes later. If your blood pressure or swelling seems off, check in with your pharmacist or doctor.

“Bendroflumethiazide, a thiazide water pill prescribed for high blood pressure, sits at the serious end of the interaction spectrum, and the ten major-rated entries deserve attention first. Most of the 227 supplements on file either stack their own blood-pressure-lowering effect on top of the drug's or deepen the potassium loss it alrea…”
