This combo product includes two antacid ingredients, aluminum hydroxide and magnesium hydroxide, whose whole job is to neutralize stomach acid and calm heartburn or an upset stomach. Blessed thistle is a bitter herb, and bitters are thought to nudge the stomach to make more acid. So on paper, taking blessed thistle could work against the antacid part of this medicine, meaning you might get less heartburn relief than you'd expect.

I want to keep this in perspective. This is rated a minor interaction backed only by anecdotal reports, not solid studies. It also wouldn't change how the aspirin or codeine work. If you take blessed thistle and notice your antacid isn't helping as well, just space them apart or chat with your pharmacist or doctor first.
