Boswellia Serrata and Acetaminophen, Chlorpheniramine, Dextromethorphan

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Boswellia Serrata interacts with whole categories of medication. Acetaminophen, Chlorpheniramine, Dextromethorphan falls into 3 of those categories, so it can interact with Boswellia Serrata in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

1 of 3

Why this applies to Acetaminophen, Chlorpheniramine, Dextromethorphan

Acetaminophen, Chlorpheniramine, Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase the levels of CYP2D6 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

This cold and flu combo contains dextromethorphan, the cough suppressant, which your body clears mainly through an enzyme called CYP2D6. In lab studies (not in people), boswellic acids appear to slow that same enzyme down. If that held true in the body, dextromethorphan could build up a bit and lead to more drowsiness, dizziness, or feeling foggy. The chlorpheniramine in this product already causes sleepiness, so you might notice that more.

Here's the honest part: this is theoretical, based on test-tube data, not real patient reports, so it may not amount to much. The acetaminophen piece isn't affected this way. Still, if you take Boswellia regularly, mention it to your pharmacist or doctor before pairing them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Acetaminophen, Chlorpheniramine, Dextromethorphan also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, Boswellia serrata might increase the levels of CYP1A2 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP1A2 enzymes.

This combo product (Coricidin) mixes a pain reliever, an antihistamine, and a cough suppressant. The interaction on file is based on lab-dish studies suggesting Boswellia might slow down a liver enzyme called CYP1A2. In theory, if that enzyme runs slower, a little more of certain ingredients could linger in your body. For this product, that could mean a bit more of the usual effects, like extra drowsiness, dry mouth, or dizziness from the chlorpheniramine, or more sleepiness from the dextromethorphan.

Honestly, this is a theoretical concern only, seen in test tubes, not in real people. None of these ingredients are prodrugs, so nothing flips here. Still, if you feel unusually groggy, check in with your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Acetaminophen, Chlorpheniramine, Dextromethorphan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combo product, Coricidin Cold and Flu, packs three ingredients into one pill: acetaminophen for pain and fever, chlorpheniramine (an antihistamine), and dextromethorphan (a cough suppressant). The piece worth noting here is dextromethorphan, which the liver partly processes through an enzyme called CYP3A4. In lab dish studies only, boswellia appears to slow that enzyme down. If that played out in real life, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or a foggy feeling.

Here's my honest take: this is theoretical, based on test-tube data, not real people. For most folks taking this short-term cold medicine, it's unlikely to cause trouble. Still, if you take boswellia regularly, mention it to your pharmacist before pairing them.

From our pharmacist’s overview on the Acetaminophen, Chlorpheniramine, Dextromethorphan page

“Alcohol is the risk to understand first. It carries major ratings alongside the acetaminophen, chlorpheniramine, and dextromethorphan combination sold as Coricidin II Extra Strength Cold and Flu, because acetaminophen already taxes the liver, and heavy drinking on top of it raises the chance of genuine liver injury.”
